Please note: Do not submit forms directly to the IRB and GCO. Instead, bring your proposal to 3 East 101st Street, Room 211, and leave it with Maggi Rodriguez (x58076). This will facilitate acquiring departmental signatures, and ensures the department has a copy, and (most importantly) lets us track the little piece of paper the GCO gives you that’s the only record of the number they assign the application (if this is lost, it’s a big hassle).
